Moorditj Koort

Moorditj Koort Aboriginal Health and Wellness Centre is a community controlled, not-for-profit organisation that supports the health and wellness of Aboriginal and Torres Strait Islander people.

We offer a number of services to the community, including the Moorditj Koort Care Coordination program that helps Aboriginal and Torres Strait Islanders manage their health and wellbeing, in particular for those who have a chronic disease.

Our aim is to be a professional, independent, all-inclusive organisation that offers culturally secure health and wellbeing services to our local Aboriginal community. We will do this by developing and strengthening the relationship between local health services, agencies and the community, and to improve the planning, evaluation and delivery of health services.
