Nic is a fully Accredited Exercise Physiologist with a 5 year Human Movement/ Exercise Physiology Degree. She has a background of elite sporting achievements where she has spent 17 years either playing or coaching elite sports, in particular Womens football and Touch football at a national level. Nic has also taught PDHPE for 10 years in secondary education in Dubbo NSW. All the above has encouraged her vested interest in exercise prescription for children with additional needs, athletes and individuals with chronic conditions.
Clint has over 15 years experience as a paramedic and senior registered nurse, specialising in emergency nursing (trauma). Clint has over 10 years experience as a health manager, specifically in practice management and operational planning.

Nic will provide rehabilitation programs completed at a gym, pool or home designed specifically to rehabilitate injuries, improve chronic conditions and facilitate client’s set goals. Each client is assessed initially, to allow development of a tailored gym, hydrotherapy or physical conditioning program.

Recommendation/Referral Pathways: This may come from a GP, family or friend who knows about the benefits of an AEP. Clients may be eligible for a Medicare funded Primary Health Care Program (usually bulk-billed sessions with the Exercise Physiologist), DVA or for Private Health Fund Rebates.

Medicare Enhanced Primary Care (EPC) Referral: To be eligible for a Medicare Bulk Billed Enhanced Primary Care Exercise Physiologist Referral you must present with a chronic medical condition and complex care needs. A chronic condition is one that has been or is likely to be present for six or more months. Complex care needs are ongoing and care is needed from a multidisciplinary team consisting of the referring GP. A referral must come from your General Practitioner. Enhanced Primary Care consists of a GP Management Plan (GPMP) (Medicare Benefits Schedule Item 21) and Team Care Arrangements (TCA) (Medicare Benefits Schedule Item 723). Patients under this scheme are entitled for up to five consultations in a calendar year. Your prescription will be geared towards your personal circumstances with either a home program or Wellness Services program. Wellness Service Program provide you with sustainable physical activity options which will help you comply and adhere to your exercise long-term.

Department of Veterans Affair (DVA) Exercise Physiologist Referral: Entitled Department of Veterans' Affairs (DVA) clients may be referred for clinically necessary Exercise Physiology treatment by their General Practitioner on a valid D904 referral form.

Type 2 Diabetes Group Education Referral: Patients with diagnosed Type II Diabetes are eligible for a Lifestyle Modification Program bulk billed through Medicare. Those that benefit most are those who demonstrate a willingness to change, are able to contribute to group processes effectively and have a potential for self management. Before taking part in the Group Sessions, the referred patient will have a one on one individual assessment to identify if the patient is suitable to the group education and for collating a comprehensive patient history, identifying individual goals and preparation for appropriate group services. Patients identified in the individual assessment as appropriate for group education progress to the eight group education sessions. In small groups of between 2 and 12 persons you will learn about exercise strategies, blood glucose monitoring, food labels and modification, associated health care concerns and strategies for change. A referral is needed from your General Practitioner, and to be eligible must have Type Two Diabetes and a GP Management Plan (Item 721 or 725) in place. These education sessions are in addition to the five individual allied health services available to eligible EPC patients.
