The Central Victorian Health Alliance PCP was established in 2000, initiated and funded by the Victorian Government with the aim to “improve the health and wellbeing of people using primary care services and to reduce avoidable use of hospital, medical and residential services”. The members of the Partnership are local governments, hospital and health services, community and women’s health services, medical practices, aboriginal health services and non-profit organisations based in or servicing the Shires of Central Goldfields, Macedon Ranges and Mount Alexander. These partners are supported by a staff of five, which assist in implementing strategies for integrated health promotion, integrated chronic disease management, service coordination and partnership development. The role of the staff is also to enhance communication between the partners. 2014 saw a name change to Central Victorian PCP which better reflects our organisational work and focus.
