39plus provides a range of services that operate with support from the Australian Governments Department of Social Services, Victorian State Government under the HACC program and various other stakeholders and enterprise activities.

As programs are aimed at meeting local needs, 39plus aims to ensure that the region retains sufficient freedom to develop separate programs to suit the clients in the area. The development and implementation of all regional services and activities are delivered with support and collaborative direction from 39plus Management.

Planned Activity Group – Day Centre
Offers a range of enjoyable and stimulating group based and individualised activities in our warm and friendly settings at our Day Centre Facilities at Eastbourne Rd. Participants are also provided with morning tea and a well balanced and nourishing lunchtime meal daily.

Coupled with skilled and compassionate staff and volunteers, our services provide security and companionship ensuring relaxing and quality experiences which encourage social inclusion and foster positive mental health and wellbeing.

Dementia Respite & Overnight Support Services
Offers short term, flexible Dementia Respite and Overnight Support Services at our well appointed respite home, Coastal Cottage, designed to accommodate the needs of older people and their carers. Coastal Cottage provides high quality care in a homely environment with professional and qualified staff 24 hours a day.

With full disability access and a secure environment, Coastal Cottage is purpose built to cater to our client’s specific needs. Services include access to a bus for activities and outings, and an extensive range of in-home activities. This may be the perfect opportunity for carers to receive a well deserved break, and introduce their loved one to respite care.

Community Outreach Activities
Provide a range of group based community outings on a weekly basis to a variety of destinations including Melbourne, Queenscliffe, Gippsland, Phillip Island and across the Mornington Peninsula.

Activities are designed specifically to assist in maintaining an individual’s ability to remain living at home by providing intellectual, physical, emotional and social stimulation. Our aim is to provide a variety of community outings that cater for all abilities and budgets.

Community Outreach Activities are available to older people or older people with a disability who are physically independent, cognitively aware, live in a community setting, able to manage a full day’s outing and do not require personal care or specialist care whilst on an outing. On occasion special outings are planned for special needs groups, namely clients with early onset dementia, to enable people with more complex needs to enjoy the benefits of this program offer.

In-home Support Services
In-home support focuses on maintaining and enhancing the physical, intellectual, psychological and social wellbeing of older people and people with disabilities. In-home support services support people to continue to live independently as long as possible, as well as providing valuable respite and support to their carers.

Activities including:
• Assistance with mobility and associated supports, accessing recreation, leisure and cultural activities, using local facilities and services
• Assistance with personal hygiene and includes general self care
• General Home Assistance services include assistance and support to develop skills to undertake basic and routine household tasks
• Respite Support – provides in home or community respite support to carers
• Overnight Care – provides a Support Worker in a client’s home for an eight hour period overnight

Medical Appointment Service
The Medical Appointment Service commenced in 1982 and is only available to frail older people and people with a disability who are 60 years of age and over, with no other means of transport, are not in residential or supported accommodation care and who live on the Southern Peninsula.

The Medical Appointment Service is managed by Volunteer Phone Operators and Drivers. The service operates Monday to Friday and coordinates transport for eligible clients to and from medical appointments and compassionate visiting. Clients are picked up from their home, taken to their scheduled appointment, waited for, then returned home. Drivers not only take clients to their local general practitioner, but also to specialist appointments and minor day procedures as far away as Monash, Box Hill and the City.

39plus also provides services which are not government funded. These services operate on a ‘fee for service’ basis and include:

Community Store
39plus operates the Community Store from its shop front located at 1641 Point Nepean Road Rosebud West (open Mon to Fri, 10.00am to 4.00pm, Sat 10.00am to 1.00pm) and stocks new & pre-loved ladies, men’s and children’s fashion along with various accessories and miscellaneous items.

Occasional Care for the Aged
39plus recognises the need for flexible occasional care services. The program aims to support carers who may need to attend a special appointment or just require a day of respite. The program operates from our Day Centre in Eastbourne Road Rosebud. The program offers group based and individualised activities in a warm and friendly environment.

Weekend Respite
Coastal Cottage is a boutique style respite home on the beautiful Mornington Peninsula. Clients are greeted with a welcoming, homely atmosphere, plenty of living areas, a large well equipped kitchen, five bedrooms and two bathrooms. There is a large undercover outdoor area for guests to enjoy a BBQ, gardening, outdoor activities, or just a quiet spot to relax.

The cottage provides transport for client activities and outings visiting various places of interest on the Peninsula. In home activities include board games, movies, cards, discussions, indoor/outdoor bowling and interactive Games.

Since its inception in 1982 as a volunteer medical transport service for residents from the southern regions of the Mornington Peninsula, 39plus, previously operating as Southern Peninsula Community Care Inc, has expanded to provide a range of community based services for older people across the Southern Metropolitan Region and in particular, the Mornington Peninsula Shire.

Still offering the Medical Appointment Service to the community as a core program, 39plus now has a suite of complimentary support services. These community based services include Centre-Based Planned Activity Groups, Community Outreach Activities within, across and outside the Peninsula, weekly and weekend supported Overnight Respite specialising in dementia specific support, Host Home Respite, and In-home Support Services.

Our services focus on maintaining and enhancing physical, intellectual, psychological and social wellbeing as well as supporting and strengthening independence for older people and people with disabilities. Our services exist to support people to continue to live independently, for as long as possible, through the provision of valuable respite and support options to carers and clients alike.

39plus is dedicated to providing the highest level of support and care and has genuine compassion and empathy to support and enhance the dignity of older people, and people with disabilities with their overall wellbeing.

All 39plus services are modelled on best practice and the organisation is committed to continuous evaluation and improvement of the governance, management and operations of the organisation.
