Services provided:
Camden Family Medical Centre is a primary health care organisation which delivers a broad range of primary health and welfare programs and clinical services. The services broadly include GP consultations, general surgery consultations, mental health/ psychological consultations and counselling, exercise physiology consultations, diet and nutrition consultations, skin checks, nurse consultations, home care and various health education/awareness programs. Additionally, the centre also aims to provide the right care in the right place at the right time to all the clients by planning and coordinating well defined referral system with specialists, other health professionals, local hospitals, laboratories, diagnostic centers, government bodies, private healthcare organisations and non-profit organisations.

Director:
Dr Kapoor is the director and also the principal G.P of the centre. He graduated in 1975 and worked in hospitals until 1981. Since then he has accumulated more than 40 years of experience in general practice . His special interests are paediatrics and geriatrics. He can advise you on various issues relating to your health.

Health Services Manager
Dr Preeti Sood (PGDHHM, MHS) manages the day to day operations within the Health Services and provides overall direction and management for the centre.

Nurses (Sherre Cooper, Kylie Skinner, Rebecca Stephenson) assist doctors with pathology, such as blood tests, immunisations and Pap Smears, as well as Health Assessments and Care Plans.

Fees and billing arrangements:
This practice bulk bills all patients except workers compensation, certain medical examinations/reports and after hour’s home visits.

Services provided:
General consultations, home visits, check ups, family planning, pap smears, pregnancy tests, ECG; heart check, counselling, vaccination; children and travel, minor surgery; stitching cuts, removal of moles and sunspots, liquid nitrogen freezing therapy for sun spots and warts, industrial medical advice and consultation, sports medicine, skin checks, blood collection and workers compensation cases.

Appointments:
It is strongly recommended to make appointments to reduce waiting time for patients and for planning of other activities by staff during the day. It is policy of this practice that patients will be accommodated on the day of their preference. Emergencies will be given priority. Staff will advise you of any delays on arrival and will attempt to minimise waiting time. If you need an extended consultation with the doctor, have complex or multiple issues you are advised to discuss this with the staff when booking the appointment.

Follow up of results:
At the time of your appointment you will be advised to arrange an appointment for follow up of results of any tests performed. Patients with results of concern will receive a call from the practice to arrange earlier follow up. Due to privacy & confidentiality, results will NOT be given over the telephone unless prior arrangements made with GP.

Recall/reminder system:
Dr Kapoor has a system in place to remind patients of routine appointments eg. Hep B injections or pap smears, results that are of a very serious nature, and also for reminders for routine health checks. Let Dr Kapoor or staff know if you do not wish to receive that service.

Consultant & General Surgeon – Visiting this practice on certain days, providing specialist surgical consultations as well as minor surgery performed in the rooms.

Consulting Psychologist – Visiting this practice on certain days, provides counselling services for children, adolescents and adults. WorkCover approved.

Policy on health information:
Dr Kapoor assures you of complete confidentiality of your medical records kept in this practice.
Only Dr. Kapoor and authorised secretarial staff have access to your records.
Your records are kept locked at all times.
Information regarding your health will not be passed on to any third party. However, with your consent, your information will be used to communicate with other health professionals such as Medical Specialists and Allied Health Professionals.

Under the following circumstances, your information will be used by staff:
• Reminding you for follow up on any appointment.
• Bulk billing to H.I.C.
• Any research purposes, accreditation purposes or for quality control.
• Legal requirements eg. Notification of communicable diseases.
• Court cases if required by law.

Your information may be used in exceptional circumstances where you are physically incapable of giving consent and your life is in danger.

Dr Kapoor and staff are willing to discuss any concerns you may have regarding your medical information.

Please ask staff to get your consent if you agree to the above information.

If you would like to put any restrictions on information, please inform any staff member.

If during the course of your treatment at the , you were unhappy and would like to lodge a complaint with staff, please do so. Dr Kapoor will discuss these issues with you. If you would like to put your concerns in writing, please do so and staff will attend to it quickly. If you have suggestions for improvement in our service, we will appreciate your comment.
